Revolutionizing the documentation process in the acute care setting, I addressed a critical problem with the accountability of data in dialysis treatment reports. Traditionally, nurses faced challenges with one-sided paper reports, leading to issues of legibility, time constraints, and inefficiencies in reporting to hospitals. The innovative solution involved digitizing the report into a PDF within the PATH app, significantly reducing documentation time and improving the overall efficiency of nurses' workflow.

PROBLEM STATEMENT

In the acutes setting, nurses struggled with handwritten, one-sided treatment reports that often led to illegible entries, time-consuming rewriting due to various factors, and challenges associated with traveling between hospitals. The monthly deadline for billing further exacerbated the situation, as nurses postponed report submissions until the last minute. This demanded a transformative approach to enhance the accountability and efficiency of data documentation for dialysis treatments.

OUTCOMES AND LESSONS

The digitized treatment report, initially designed for the PATH app, emerged as a game-changer. By allowing nurses to input data directly into the app, we streamlined the documentation process, saving valuable time. The success of the report template spread by word of mouth among nurses, leading to its adoption across DaVita. Stakeholders and business teams recognized its value, resulting in widespread use for clinics, patient-facing applications, and practice coordination. The template not only facilitates data entry but also provides clear sections for comprehensive data digestion, improving the overall reporting experience for healthcare professionals. This journey highlights the transformative power of user-centric design in solving real-world challenges and fostering collaboration across healthcare teams.
